BLET Student Application Checklist

Below you’ll find a list of the forms you’ll need to complete COA’s Basic Law Enforcement Training (BLET) admission packet. Return all forms to the BLET office in COA – Elizabeth City, room FC 129A.

☐ Transcripts: COA requires an official transcript from high school and college(s) once you have submitted your online application to the school
COA Online Orientation
☐ DD-214, if military (Member 4 form)
☐ Proof of education (copy of diploma or college transcript will be accepted for this packet only)
☐ Driver’s License copy (North Carolina license is required)
☐ Birth certificate copy
☐ Placement test – Reading Comprehension
Must take at COA and receive a score for 10th grade or higher

☐ Grade release sheet
☐ Student insurance responsibility sheet
☐ Accommodation dorm (in office)
☐ F-1 Medical History Report (North Carolina physician)
☐ F-2 Medical Exam Report (North Carolina physician)
☐ F-3 Personal history statement
☐ Sponsorship letter (North Carolina law enforcement agencies only)
☐ Sponsorship letter and letter of payment
☐ Certified background checks:

  • You must supply an original criminal background check from all residences (county/city/state) since age 16
  • A military criminal background check is required, if applicable
